Learning outcomes
3. Coordinating Care – Level 1
LOL 3.1 – RVZ – N1 – Authentic (VLPs & DT)
The student works within a moderately complex authentic context, partly under supervision, based on the principles of person-centred and family-centred care and shared decision-making, and utilises the input and involvement of the care recipient and their loved ones to encourage self-management.
LOL 3.2 – RVZ – N1 – Authentic (VLPs & DT)
The student, partly under supervision, within a moderately complex authentic context, prioritises the interests of the care recipient and their loved ones by collaborating intra- and inter-professionally with partners to ensure the continuity and coordination of care
Indicators
Description
3.1 Follows the steps of shared decision-making in a methodical and culturally sensitive manner when coordinating care between the care recipient and a loved one (BN2030).
3.2 Demonstrates a supportive role in the shared decision-making process, paying attention to the interests of the care recipient and their loved ones (BN2030).
3.3 Assesses the level of self-management of the care recipient and their next of kin, taking into account the care recipient’s vulnerability (BN2030).
3.4 Identifies different perspectives from the care recipient and their next of kin, colleagues and other professionals (BN2030).
3.5 Identifies their own role and responsibilities within intra- and interprofessional collaboration and can distinguish their own discipline from other disciplines (BN2030).
3.6 Carries out basic reporting and handover procedures (BN2030).
Content
In this course, students develop the skills needed to ensure continuity of care for their own care recipients.
Students learn to understand their own role as a nurse within the healthcare system, including relevant disciplines and legislation. In doing so, they develop nursing leadership skills. Students enhance their knowledge and skills relating to care pathways, interprofessional collaboration, and the coordination and management of care.
Students improve their communication skills by applying the basic principles of shared decision-making and person- and family-centred care, demonstrated through simulated conversations and in practice.
During the sessions, students gain insight into their own learning process and the steps they need to take to achieve the learning outcome. This is achieved, amongst other things, by giving and receiving feedback.
Topics that may be covered:
- Person- and family-centred care
- Interprofessional collaboration
- Evidence-Based Practice (EBP)
- Nursing leadership
- Healthcare technology
- Legislation and regulations
- PPO
- SDG17 (Partnerships for the Goals)
- Intellectual disability
Competence Mastery Level 1 (CBN, 2022)
At the first level, the context is of medium complexity, with the focus on the methodical application of knowledge and skills in accordance with guidelines and protocols in varying but similar situations. The nature of the task is structured, involving the application of familiar methods. The student plays a supporting role within collaborative settings.
The student learns to manage their own learning (self-direction) in terms of planning, motivation and reflection. Reflection is aimed at knowledge and understanding, with a focus on innovation.
The guidance provided is geared towards increasing self-direction.
Assessment:
In this course, the student’s competences are assessed on the basis of specific learning outcomes, with the indicators serving as a guide and support for the assessment. The assessment consists of a portfolio assessment. Feedback is provided on the individual pieces of evidence for the portfolio. The portfolio assessment is marked using a rubric with a grade and must be passed.
- TEST O1 – RVZ – N1 – Coordination and joint decision-making
Evidence for the assessment
Simulated communication session: joint decision-making
Individual assignment
A recording is made of a simulated conversation between the student and a simulated patient, based on a case study provided. In this conversation, the student demonstrates the methodology of the basic principles of shared decision-making. In doing so, they take into account the interests of the care recipient (and their relatives). The care recipient’s level of independence is assessed, and their vulnerability is taken into account.
Coordination of care
Individual assignment
- Describes a patient journey, drawing on theoretical underpinnings, with a focus on the preparation, reporting and handover of intra- and interprofessional consultations.
- The student presents his or her findings during the teaching session
Short Practical Evaluations
Individual assignment
Short practical evaluations by those involved regarding performance in relation to the learning outcomes associated with the module, which are independent of the specific study pathway.
